CANCELLATION POLICY

If you need to cancel or reschedule your appointment I require at least 48 hours notice prior to your scheduled appointment to avoid a late cancellation fee.

PAYMENT FOR SERVICES

I accept debit/credit card payments (using Stripe application). Payments are due at the end of each session.

Options for payment are self payment on the Simple Practice Website or you can ask to be signed up for auto-billing if you don't want to worry about paying after each session. If opting for automatic payment your card will be ran at midnight after your session.

If opting for self payment, unpaid invoices will be considered past due after 3 days. If financial burdens occur I am open to discussing alternative timelines for bill payments on a case by case basis if you let me know.

INSURANCE

I am currently in network with Premera, Regence, Aetna, First Choice and Cigna, *OON.

For those not in network with these insurances, payment in full is due at the end of each session. I can provide you with a monthly receipt/superbill upon your request that you can submit to your insurance company for reimbursement from *out of network benefits. Check with your insurance company to confirm if they will reimburse you for *out of network providers.

I would recommend calling the number on the back of your insurance card and checking your specific plan to understand what your deductibles are, copayments/coinsurances and other specific details are for you so that you will understand what your responsibilities are.

It can be helpful to clarify your with your insurance prior to meeting so you can understand your specific plan's coverage because plans can vary a lot on specifics and it is never fun to be surprised with unexpected bills.

Some possible questions that can assist with this:

  • if you need to be aware of deductibles or co-payments and what these are specifically for behavioral health

  • If you need prior approval for services and what is required

  • the rate of coverage for behavioral health services

  • when your deductible renews in the year (*This often means you may have to pay the deductible amount again before coverage resumes)

  • how and what they reimburse for seeing out of network providers and clarifying if oon benefits have a separate deductible

GOOD FAITH ESTIMATE

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost for any non-emergency items or service. This includes related costs like medical tests, prescription drugs, equipment and hospital fees.

  • Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or a service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or take a picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
